Subtask lifecycle incomplete: no API to mark done / update / delete

Description

Contract has createSubtask + list + get only — done:false is permanent; no PATCH/toggle, no delete. Blocks real subtask usage (JC 2026-08-23 wants phase-level subtasks per item, e.g. P6.1-P6.4 under BUILD-7 — currently their status lives in title text, which is wrong). Fix: PATCH /subtasks/{subId} {done, title} + MCP tool + web checkbox; emits subtask.updated activity (Slack dispatcher formatter needs the new event type too).
