Item doc links: attach + render S3-hosted markdown files

Description

JC 2026-08-23: items should link to MD files hosted in S3 (specs/design docs). Scope: item.docLinks[] (title + s3:// or bucket/key ref) via the new PATCH item; item detail renders linked MD inline (server-side fetch — astroFn gets scoped s3:GetObject on a docs bucket/prefix; NEVER presign to public); safe renderer (mdLite may need tables/headings/code-fences upgrade); MCP tool parity so agents can attach docs. Depends on BUILD-31 G1 (PATCH item).

**Scope addition (JC)** - Not just linking pre-existing S3 files: UPLOAD from the item page + auto-associate to the item - Upload path: authed same-origin endpoint issues presigned PUT into the docs bucket (item-scoped prefix, size/type limits) — key never client-side, anon cannot upload - Attachment record lands on the item via PATCH; MD renders inline, other types listed w/ download - Decomposed into subtasks U1-U3
